Everything comes together in an individualized treatment plan covering what your child needs and what we recommend — nothing more. You'll understand it, and the reasons behind it, before you leave.
Built on trust
A strong relationship between family and provider is the heart of good care. We encourage parents — and older kids — to share every concern as it comes up, and we'll monitor progress with periodic follow-up visits to make sure the plan is working as intended.

Your child's PPO insurance card, so we can help you make sense of your benefits, as well as your ID.
No Forms! (Done Ahead)
In the days before the visit you'll get a text with a link to complete the new patient forms online — nothing to print or bring.
If Applicable...
If any of these are applicable, please bring your referral form, medication list, consent (from guardian) to bring a minor if you are not a guardian, and/or divorce custody paperwork.

The appointment would be pretty short without them! But seriously, please also bring any questions you may have — we'll make time for all of it.

No printing, no clipboard. In the days before the appointment you'll automatically receive a text with a link to complete the forms online through Weave — a few minutes on your phone, and check-in is effortless.
